Transcript Request Form

Please fill out the following form to request your transcripts. The fee to process transcripts is $5.00. After submitting the form, you will be prompted to pay the $5.00 fee. We will not begin processing your request until after your fee has been paid. Once you submit your request, you have up to one week to pay. After that, your request will expire and you will need to fill out the form again to receive your transcripts.

If you are a current student and/or under the age of 18, you must use the transcript request form that is in the main office.
